DA Form 2166-8 (NCOER) although some states use a national guard specific format like Minnesota (MNARNG Form 2166-8) which is a modified version of the DA Form 2166-8 (NCOER). It is possible that the National Guard has switched to the DA Form 2166-8 as the MNARNG form is quite difficult to locate.
A "Complete the Record" entry is used on an NCOER when a portion of the rated period was previously not covered by an evaluation report, but now needs to be included to provide a comprehensive assessment of the rated NCO's performance.
