This depends on the type of surgery & stage of pregnancy. If the patient has unsafe ear disease with complication (of CSOM) like cranial abscess, meningitis etc. or malignancy then surgery has to be done inspite of stage of pregnancy.

If there is benign ear disease like safe CSOM, Otitis media with effusion, then surgery may be delayed.

Generally second trimester is the safest time for undergoing an elective surgery.

Yes but the jewellery will need to be removed to prevent the possible chances of cross contamination from the jewellery, at which point the piercing will shrink over the few days the plug is out.
