It was not conclusive that Daryl Hannah's Elle Driver character in Kill Bill died. Uma Thurman's Beatrice Kiddo character defeated her in the fight by removing Elle's remaining eye. Elle was left thrashing helplessly in the trailer, and Beatrice just left. It is possible that Elle was bitten by the Black Mambo still left in the trailer, but this was not shown explicitly.

Elle was clearly traumatized after being shot by the unsub in "The Fisher King", she returns to work in "The Aftermath", a case about a man who comes into women's homes and rapes them. She goes undercover in an attempt to catch the unsub in the act, but she panics and makes the arrest too early. Since she did so, they must release him seeing as how they don't have any evidence. Elle goes to confront the unsub and winds up shooting him. The stress becomes too much for her, and she makes her final appearance in "The Boogeyman"
Thomas Gibson was in a physical altercation with a co-executive producer and writer on the set for Criminal Minds. During the altercation, he kicked the writer. This is after a previous incident that he was required to attend anger management classes for. He was initially suspended for two episodes for this incident, but it was later announced that he was fired.
Mandy left the show because it was so emotionally draining and after a few years, he wasn't able to shake the dark subject matter when he left the set. Mandy did not let any of the cast know he was struggling with it because he didn't want them to worry about him. He was very supportive and kept the mood light on the set, so it was a surprise when they came back from hiatus and he was gone.
