The needle does not really hurt, but the fluid getting pushed into you can hurt a little. In either case it is probably no worse than being pinched hard by someone.
Yes they do hurt because the are basicly putting a pin with a open end into youre arm etc. If u get numb cream before they inject you, then you wont feel a thing trust me i am terrafied of them but...
