This depends of the country you live in, and even the province or state. It also depends on the policy you have, even within the same provider. The best thing would be to contact your local dentist, or the dental section of your hospital, or your insurance provider and ask them.

There are also dental plans that are not insurance, but offer reduced rates that might help reduce the costs of extractions.

It depends on your carrier. Mine covered the two impacted teeth but not the two very decayed not impacted teeth. Your oral surgeon should be able to tell you in the consult.

No. The only exception would be if the tooth needed to come out for a medical reason such as cancer, but that's about it!
