This is the uncommon practice of excommunication. It literally means separating someone from communion (not allowing them to take communion, thus preventing entry into heaven).

It is uncommon because of the stipulations behind it, namely that the individual needs to be spreading confusion or dissention among faithful followers of the church. This makes it almost necessary for the individual excommunicated to be public figures or politicians.

For the reason explained above, the question including "because he or she committed a sin," makes it hard to answer, becuase almost all churches, even in that dark time, allow for some sort of redemtion.
