It has long been thought that Joaquin was born with a cleft lip and the scar is the result of corrective surgery. This is not so. Rather, Joaquin was born with the scar. The scar developed while Joaquin was in his mother's stomach. It is known as a microform cleft, a mild form of a cleft lip. A "microform cleft can appear as small as a little dent in the red part of the lip or look like a scar from the lip up to the nostril," as in Joaquin's case."

IMDB "Walk the Line" states Joaquin Phoenix was hospitalized post filming WTL due to dealing with similarities with his life and Johnny Cash's. Specifically the passing of his brother River and Johnny Cash's brother. I suspect it was a mental hospital or psych ward due to the nature of the hospitalization. I think it could very well be a possibility of depression and post-traumatic-stress disorder.

During the scene where Cash learns of his brother's fatal accident in the Saw Mill factory, Phoenix started to bang his head against the bed post. He said he was getting into the role. I just know he was sent to the hospital for observation.

This was 2005 and years before "I'm Still Here"

Joaquin Raphael Phoenix was born in Puerto Rico on October 28th 1974.

He had an older brother, River (b.1970) has an older sister, Rain (b.1972) and two younger sisters, Liberty (b.1976) and Summer (b.1978).

He born into the christian group (or cult) The Children Of God and his family stayed with them until he was 3.

The same year, he and his family became vegan.

He, with his family, moved back to the US in 1978.

The same year Joaquin chose to change his name to 'Leaf' in order to have a name more like his older siblings River and Rain.

In Los Angeles he and his brother and sisters would perform a family musical act on the streets on LA for money.

He, along with his brothers and sisters, were signed with a talent agent when he was six years old.

He had a substantial career as a child actor, appearing in numerous tv shows as well as starring in 'Spacecamp' (1986), 'Russkies' (1987).

Phoenix and his family moved to Gainesville, Florida when he was 14.

He starred in the movie 'Parenthood' (1989) shortly after.
