It will hurt like hell, blood will come out rapidly (possibly with enough force to push the plunger back automatically), and you need to immediately apply pressure and treat the wound or you could be in some serious trouble.

Arteries have thicker walls and are deeper, so you will most likely know if you hit an artery instead of a vein. Do NOT inject into your artery, this is very dangerous.
