Coronary artery disease is usually caused by atherosclerosis.
Basically Cholesterol and other fatty substances accumulate on the inner wall of the arteries over time causing plaque build-up and eventually blocking blood from flowing freely through the heart. A combination of your genes and diet (high in red meat/cheeses and other saturated fats) can be the cause of the disease as well as aggravate it.
On the other hand exercise, weight loss and diet can also reverse the effects... all of which Tim Russert had been doing in the time before his death. So it is a matter of the individual and luck.

Tim Russert died at the age of 58 from asymptomatic coronary artery disease. The autopsy revealed an enlarged heart and atherosclerosis and the terrible clot that caused the heart attack.
