In North Korea, the leadership is a hereditary dictatorship (Complete power from parent to sibling). Leaders are chosen by the current leader before their time of death. For example the current leader is Kim Jong-il, and he hasn't formally announced it (But it is speculated to be true) that his heir is Kim Jong-un. Hope this helps!
