How are photographers paid by authors who use their photos in a newly published book?

The author and/or publisher negotiates this with the copyright holder.

Often a publisher gives an author a budget for the illustrations. In some cases, the copyright holder may charge nothing and in other cases the fees may be surprisingly high, so it is useful to find more photos than one intends to use. Occasionally, there may be other complications. For example, if you're unlucky, copyright ownership may be in dispute in some cases.
