It is possible for a human to be a true hermaphrodite. Due to a discovery in Texas in 2003, it has been proven that a single human being can possess both male and female reproductive organs inside and outside the body. It is believed that a DNA disorder called Chimerism is the source of this condition.

The first step would be normal hormonal transition to female. From there you need to find a doctor willing to perform the surgery, most will have issues with not transitioning you all the way.
The procedure you will need are not those used for transexuals, but those used to aid women with Vaginal Agenesis (congenital lack of a vagina). You will not get the sensation in your new vagina that a MtF transsexual would get in hers, as they carefully more nerve endings for penile inversion, and you would have none to move.

McIndoe - uses a skin graft wrapped around a foam and rubber plug while it heals. The plug is removed after healing is complete.
Vecchietti - minimally invasive laproscopic technique. In which they pull an olive shaped stint into the body, using a thread connected between the olive at your neovagina and a ratcheting pulley at your naval. Over the course of a week or more, the ratcheting devicing is tightened once of twice daily, causing stretching and microlacerations (a good thing)
Davydov - minimally invasive laproscopic technique. A section of the peritoneum (lining of the abdominal cavity) is reshaped to form the lining of the neovagina. a silicone gel insert is inserted for the duration of the healing period.
In all cases, vaginal dilators are to be used for 30 minutes a day until the patient becomes sexually active. In this case specifically being penetrated, not doing the penetration on someone else.
