Sounds like there is a desperate need for re-training! Easiest way to accomplish this, I think, is to keep it crated at all times unless she is directly in your line of sight . Take her out frequently and heap praise on her every time she 'performs'. Dogs will rarely mess where they sleep, so normally they will not do so in the crate. Do not allow freedom to roam the house at all. As she gets better at taking care of business when she is outdoors you can gradually increase the amount of freedom inside the house you give her. Crating is NOT cruel or harsh. Most all dogs quickly become accustomed to it and my dogs, in fact, use their crates daily. I just leave the doors open nowadays...they retreat to it when they need a bit of quiet, to nao, and to sleep every night. They willingly go in. It provides them a place, a 'den' if you will that is their safetey nest. And it is the simpliest way to housebreak and control your dog. Good luck!
