It's in reference to the shape of the city before the expansion of the city's suburbs, but particularly prior to the city's boundary expanding up-river into uptown New Orleans which was then part of Jefferson Parish. The city was built primarily around a bend in the Mississippi River because it was the highest ground. The French Quarter was the top of the crescent.

According to the 1938 New Orleans City Guide, "The Crescent City," is derived from the fact that the site of the original town (Vieux Carre') was built on a sharp bend of the (Mississippi) river."
