The people of the Middle Ages were not dirty. They were actually very clean, because they believed the condition of a person's body was a reflection of how he kept his soul. They also believed that bad air was a disease vector, and so they tried very hard to eliminate odors, which they did by cleaning and scattering flower petals. They got their reputation for being dirty from Renaissance types who forgot soap but remembered medieval people did not have ready access to perfume.

I assume you mean 'dirty' literally and not figuratively, in which case the medieval period was pretty filthy. In cities, the streets were packed with humans and animals alike, and trash, waste, excrement and even dead bodies were everywhere. Most people also rarely bathed, if ever. This lead to disease of all sorts running rampant.
