In Canada and the USA mayors may be of either sex. A formal introduction would be "The Honorable (First and second name), His/Her Honor the Mayor, Mayor (Name), Mr./Madame Mayor, Your Honor. Informally or privately, and depending on your level of familiarity, by first name.
The spouse of the Mayor should be addressed as M./Mrs./Miss/Mme (Preferred Name). Never use "... and his/her wife/husband (Name)"
