How do you appeal a decision made by the court of appeals for 'reverse and remand'?

Answer:
When a case is remanded (returned) to a lower court for disposition, you can't do anything until that court rules. If you are still unhappy with the decision, or the decision doesn't meet the criteria detailed by the appellate court, you can go back to the appellate court that ordered the case remanded.

If the response to the above steps remains unsatisfactory, you can appeal to the next higher court, which would be the state Supreme Court. If the case involves a preserved question of federal or constitutional law, it may be eligible for appeal to the US Supreme Court, once all the state appellate options have been exhausted.
