The answer is yes, I recently blew out a pair, yet the circumstances were more than most people would have to worry about. I managed this feat by while recording drum tracks for a cd, the beats were plugged into a headphone interface that boosted the volume output by a crazy amount. This allowed the headphones to get louder than any music device would allow them to get and therefore blew them out.
As far I know unfortunately you can't, but i have heard from a couple people that if you put the headphone in your mouth and breath hard and suck on it that it will temporarily fix this problem
Headphones weren't meant to be blown out, so it is rather difficult to do so. Playing music at high volumes for long periods of time will gradually wear out and blow out the headphones.
