No, Combat Flight Simulator 3 does not need a Flight Simulator.
Shift+c ejects the canopy, and pressing O three times bails out if you are high enough.

I use the Logitech Attack 3. You can get it online for less than $20. The only problem I have with it is that it does not rotate, so you will need to use the num pad for rudders.
Not really, while the program loads and runs, it seems incompatible with any joystick.
