Birth certificates are usually issued by the state in which you or another person is born. You can contact the state's clerk of court or the Bureau of Vital Statistics to acquire a certified copy of your own certificate. Even though it is a replacement birth certificate, it would still be an original certified copy.

In order to obtain a copy, you must turn in an application that includes your full name, you parents' full names, the date and location of birth, and the purpose of your search. You must also send them a copy of your legal identification which should be updated and current.

Another view:

In many jurisdictions, it is the city or county of your birth who records such statistics and maintains those records. You should first try contacting your County Dept. of Health, or Bureau of Vital Statistics, or similar office, to determine if they are the office with which you should be dealing.

You cannot actually get a "new" birth certificate through this process - you can only get a copy of your original birth certificate.

Not unless you can show ABSOLUTE proof that the records were initially entered incorrectly.

Addded: (in the US) you may NOT ever change your ORIGINAL birth certificate, the only thing you can do is amend it, via court order, using the same criteria as mentioned in the original answer.

You can often order your birth certificate online from the state or county health department (or whatever agency handles vital records) where the birth occurred. You will have to be an authorized person to obtain a copy if it is not for yourself. The rules for obtaining birth certificates vary from state to state. You will have to show proper identification and pay a small fee. There are also third party vendors, such as VitalChek that you can order from for additional fees.
