what i Do is i get toothpaste MAKE SURE IT IS NOT TEXTURED OR IT WILL SCRATCH IT MORE!!! put a bit of it on and rub it around the whole disc, let it sit for 5 mins and wash it all off with running warm water.=, then wipe it off with a soft cloth and try :D

Check the back of a disc there might be a scratch on it. If not it might just be your PlayStation is having trouble reading the disc. Finally maybe your PlayStation is having problems.
