∙ 12y agoLet's say you are on Orange. Phone 07973100450 and enter your phone number when prompted. Press 2, then 2. The system will ask you to enter you mobile number again for security. You should then receive your PUK. Write it down or enter it straight into the phone, as the automated voice speaks fast. Hope this helped...
