There are probably many reasons but a start would be to remove the speakers and check them for connections. If the vehicle is older there could be errosion depending on the speaker placement and the type of vehicle. You could also look at the connections at the rear of the head unit or a possible break in the speaker wire going to the speakers. There could also be a head unit (cd player, tape deck, etc) problem like a "burnt" channel or wire harness problem.usually start AT the speaker and work your way forward.
if your mp3 player has an external speaker then no you will not need headphones. If there is no external speaker than yes. I recommend skullex brand. They have built in subs and are the best quality for their buck. You can also find docking stations that act as speakers for your mp3.
I had to replace the ones in my 1997 Sebring. Using a tool such as a flat head screwdriver detach the plastic cover that is over the door speaker. Remove the screws that are holding the speaker to the door. Pull the speaker out enough to detach the plastic clip that connects the wiring to the speaker. Installing the door speaker is the reverse of removing the speakers.

Could be a short in the factory wiring...i am having the same issue only reverse...my rear function fine but i get no sound out of the fronts...I would hard wire them myself...get some speaker wire run it from the speaker to head unit, hide the wire like you would if installing an Amp...get the wiring diagram for the stock speakers and splice your new wires into the factory system as close as you can to the head unit...if it still dosent work the head unit may have some bad channels

If you open the door there will be a star head bolt in the door jam on the speaker grille, you just unbolt this and it should come right off.
