you need to setup a worthy site then submit it to adsense, there system will review the content and if it doesn't flag up any alerts then you will get approved quickly, if you already have a site then just submit the URL of that site, adsense apparently prefers domain level site such www.exmaple.com, i don't think they are keen on sites that are within the directory of another site www.example.com/cool/site/index.htm
