The easiest way would be to take the Uptown(Manhattan-bound) 3 (of the 1-2-3, the red line) from the New Lots Station. The New Lots Station is at the corner of New Lots Avenue and Warwick Street. Warwick Street is two streets west of Cleveland Street, and New Lots Avenue is at the south end of both Warwick and Cleveland Streets.

Take the 3 to Atlantic Avenue, and transfer at Atlantic Avenue to the Downtown N train (of the N-R-Q-W, the yellow line) at Atlantic Avenue-Pacific Street (the Atlantic Avenue Station and the Atlantic Avenue-Pacific Street Station are connected to each other).

Nostrand Avenue is a very long avenue. It runs through almost all of Brooklyn. So, which train to take depends upon where on Nostrand Avenue you are coming from. So, unless you were referring to the Nostrand Avenue Stations, please ask your question again, this time specifying the cross street on Nostrand Avenue that you are starting out from.
