Well you couldn't really blame the girl for misconstruing it's meaning, as people don't often get the names of living siblings tattooed on them. However, I would incorporate a family tree idea most likely for it to be plain what its meaning is. You could do this, or do an inscription above or below the name that would give an indication that it is your sister's name.
