Place the plug-in into the Photoshop plug-in folder that is created when you install photoshop ( don't forget there are different plug-in folders, so choose the correct one ).

Alternatively you can create a shortcut from that plug-in stored elsewhere on your computer and put the shortcut into the folder. The plug-in will be shown when you next restart photoshop. For example, if you wish to add the free plug-in "ICO (Windows Icon) FORMAT" -- downloadable here -- http://www.telegraphics.com.au/sw/ ..., and the file format is ICOFormat.8bi, once extracted; copy this file into your Adobe Photoshop Plug-in folder and look for the File Formats folder. It is usually located in this path -- C:\Adobe\Adobe Photoshop CS4\Plug-Ins\File Formats. Then restart Photoshop.

Edit > Preferences > Plug Ins.. Usually plug ins are listed in Filter menu or Window > Extensions.
