I underwent Tommy john surgery 3 1/2 months ago. i was diagnosed as a partial UCL (Ulnar Collateral Ligament) tear in December and tried to rehab the injury for two months, when i got back to throwing i retore it and knew that surgery was the only option. if you have not yet seen a doctor, i would recommend doing so... some of your symptoms would be constant soreness and pain on the inside of the elbow with overhead tasks like putting on a seatbelt, pain when fully extending or retracting your arm, sharp pain when trying to throw, and the area where the UCL is located will be sore or painful when pressing on it. if you are still able to throw with mild pain you will probably experience a loss in velocity. I experienced this but kept throwing because i am stupid, after continuing to throw with this pain it eventually became unbearable and i had lost about 5mph in velocity. Lesson Learned: if you are having pain in your elbow stop throwing immediatly and see a specialist. if you are diagnosed with a torn UCL and are considering surgery i would personally recommend trying to rehab the injury first with rest and physical therapy.
one last thing: if you are seriously thinking about this understand that the hype about gaining velocity after surgery is not true, pitchers who gained velocity did so because of the rigorous physical therapy you must do in the 4 months after surgery, not the new ligament. I can tell you from experience the physical therapy is not fun.
