Most of the time you can't. Dwarf Puffers are the only species I know of off the top of my head that can be told apart by eye.

With Dwarf Puffers- Males have "wrinkles" around their eyes and are brighter color then the females.

Mouth brooders will have an enlarged mouth, most of the time you can see from that, but all fish are different, and remember many fish lay eggs

Puffer fish amplify there mass when frightened. If it is constantly swollen, it may be pregnant. Puffer fish also will have yet tighter skin.
