You would cook it the same as any other. However if it is still frozen, you would want to defrost it first. Place the ham--still wrapped in plastic--in a bowl of water for the fastest defrost. A small ham will take 2 to 3 hours if you change the water every 30 minutes, longer for a large ham.

A refrigerated ham would take about 15 minutes per pound to cook at 350. A meat thermometer would be the best way to tell if it is done. See attached website.

All you do is put it in a baking pan with a litte bit of butter and herbs to get that smoked flavor and let it bake for 1 hour on 150. Make sure the center of the ham has been at least 75 degrees centigrade for some time to ensure it's safe to eat. If your ham is thick, it might take longer to cook.

Serve with BBQ or smoked sauce.

Thaw well, slice, reheat on flat top, griddle, pan, or oven. Microwaving will make it tough and lose the flavor. not slicing it will promote an unthorough reheat. if put in oven keep in pan with some water in the bottom and tin foil up

I wouldn't freeze whole cooked ham in a domestic (home kitchen) freezer; it takes too long to freeze and too long again to defrost, which isn't necessarily safe, and then on defrost you have to use the whole lot the same day.

Instead, freeze sliced ham or lean trimmed bacon. Layer slices between freezer paper and store in a freezer bag with the air well squeezed out and put that in another sealed freezer bag (to help prevent frost). When you want ham or bacon for your eggs or a sandwich, heat a little oil in a pan to medium-hot and take out as many slices as you need. Don't defrost! Have a sheet of newspaper handy, drop the ham into the pan and cover immediately with the paper because it will spit.

Once it's stopped spitting reduce heat and let it cook until nicely browned and crisp and use as you wish. The rest of your slices will be there in the freezer ready for next use.

When you've frozen slices of ham or lean bacon stored in the freezer you've always got an instant meal, even if you just add bread and lettuce. They're great for all kinds of cooking, from burgers to casseroles. Easy to chop while still frozen, you could simply toss some with pasta in a pan. Just remember, put the ham or bacon frozen straight into the pan; don't let it defrost.

thaw it in the refrigerator. 300ish degree oven until it's warm (varies due to ham size).

When would you glaze a cooked gammon that has been frozen?

Obviously after it has defrosted. Depending on how you intend to serve it - not whole if it is already cooked. If sliced, then a coating of glaze would be sufficient while you reheated the gammon. If attempting to reheat whole, then the glaze should be applied about 15 minutes before the end of the cooking process, but be aware the ham will be dry.
