I just did this on a '99 B2500 to replace the mirror. This truck has manual windows and mirror . . ..
1) remove the two screws behind the door handle. 2) you can now remove the black panel that the door handle is part of 3) remove the screw that was below this panel and the screw at the bottom left hand corner of the door 4) pop the cap off off the window crank, this reveals a hex bolt that holds the crank on 5) once this removed you can just lift the panel up and it comes off easily
went back together really easily. whole thing took ~30 min including retrieving a bolt I dropped down in the door, doh!
