The wall behind the tile takes so much damage anyway youll probably have to replace the Plaster,rockboard,drywall,or whatever they used,so id take a diamond blade cutting between the studs set the depth so you don't hit pipes wires,ect it will come out in sections, then ya just have to get it off the studs use a prybar.and a hammer

i am doing a project like this myself and i recomend a crowbar, because when you make a hole through the drywall(see above) it will weaken the streangth of the tile over,and so on

With a hammer and chisel or a bolster. or you can you a SDS hammer drill that has chisel feature on it. depends how much you have to do. A small bathroom could probably be done by hand without too much effort. although if they're very well adheared then hiring the correct power tool may save time and effort.

It is attached with mastic, the same as the other tiles. you MAY be able to cut the surrounding grout away and wiggle it loose. If it was attached correctly, the backing will come with it. You could be creating an even more expensive repair.
