Very easy! Call 1888-558-5500 select the option for stolen or damaged card and they will connect you to a service rep. Tell them your situation and they will send you a new card that you will receive within 5 to seven days.

∙ 12y agoGo to ur local HRA office for a referral to go downtown bk and get the replacement card r get the address for the replacement site downtown Bk. I believe u don't need a referral anymore. good luck
