It just pulls off. There are 2 indentations on either side of the ball that the ends of the hoop fit into.
It is better to start off with a normal piercing (just like spikes), since your lip will firstly try to heal and will get bigger. If you start off with a ring there's a risk that when you lip get bigger the ring will stuck in it since the lip will have limited space to expand. It is recommended to switch to the ring a month after you started using the piercing.

Depends on the size of the ring. I had a 14 gauge in my lip and you can not see a scar. on the inside of the lip i can feel where it used to be but small. outside is not seen since i have a goatee but if it does yu can barely see it.
If you are unable to get the ball off using your fingers, you will have to use pliers to get the ball off. It is best to have someone else use the pliers and not try to do it by yourself.
if you have an old barbie or brat bracelet. snip a good size piece off that depends on the size of your lip. slip the almost circle on your lip!

To get a ring in your lip.
The ring is scaled (sized) to your lip, it's not a one size fits all affair. Your professional body piercer will assist you in selecting a ring that will suit your lip. Be aware that your lip will swell so the ring must allow for swelling.
Most lip rings are in the shape of a hoop and have a ball as a closure. Simply take the ball off and gently work the space in the hoop to the hole. (if the ring is too small you may have to bend the hoop a bit to make it larger)
