How do you take the state board exam for a certified nursing assistant?

I am trying to find out if I have to go through a initial training course at a local nursing home, or can I take a refresher course on line, ( which I received a certificate from) and still go for state boards? I have been in the profession of a nursing assistant, for many years, but never got certified.
