Heroin affects the liver of course...but damage that is done by heroin is relatively small (compare to alcohol). Heroin is very quickly and effectively metabolized and doesn't affect liver function in a big way. Heroin itself is also non-toxic to liver or any other internal organs (again compare to alcohol). However variety of additives and dilutants that street heroin is "cut"with can cause after prolonged use of IV heroin (decades at least) some damage to the liver.

Heroin effects ALL the bodies systems, not just two. Your nervous system due to the addictive, sensory, neurological, and synaptic changes it has. Your cardiac system due to the irregular heart rates and vascular changes, which is why those who use heroin sometimes die of heart attacks. Your hepatic/liver system due to the toxins that it contains and the fact that it can cause liver failure. If your liver is compromised you can't remove toxins/poisions/wastes from your body, which means so lots of maintenance medications for the rest of your life or death . Your gastroentestinal system due to the fact that heroin addiction causes decreased appetite and general poor nutrition, which means your body has nothing healthy to sustain and repair itself with. Your Renal/Kidney system due to the fact that your liver isn't working so you can't filter out toxins and wastes, so these clog and destroy your kidneys. Which means dialysis for the rest of your life and medications, unless you can get a donated kidney. Your Dermal/Skin system due to the fact that all the other systems are failing so you end up with bad skin infections, sores, jaundice, etc. Hope you get the idea, but ALL and ANY of your bodys systems can not only be effected by heroin, but can be destroyed by heroin.

The majority of your organs are damaged by drug abuse.

But your liver, heart, lungs, and brain are damaged the most. The liver filters toxins from your blood, which build up and kill the organ. Drugs have varying effects on heart rate and blood pressure, which stress the heart and can lead to a heart attack. Drugs that are inhaled deteriorate the lung tissue, which makes it inefficient at exchanging oxygen and can lead to cancer. Drugs also produce a euphoric high which causes cell death in the brain, which can lead ultimately to brain death.

Your lungs, they are dialated and allows a better flow of oxygen, while at the same time, if smoked, sharp crystals unnoticable damage micro tissue in the lung, throat and mouth. Skin texture is changed and weakened system wide, and allows for easy inflamation, irritation, and damage. Muscle tissue is toned system wide, and eaten away in long term users due to lack of food. Stomach, Slow down, to near pause of the digestive system, which stops hunger. Nervous System, anaesthetic effects, Nerves are less like to send pain responses. This, plus change in skin texture is bad when someone wants to pick at sores on their body. There will be no pain resposne and they can possibly dig a big hole in them selves without notice. Bone Damage system wide do to calcium deprivation. Brain, releases excessive adrenaline, which causes damage to receptors. Releases excessive dopamine, and causes euphoria while still damaging receptors. Releases Serotonin and damages serotonin receptors. Effects logic and priorities.
