No, they do not and can not differentiate between heroin and other opiates, such as hydrocodone or morphine. Opiates all show up under one panel ("opiates"), although oxycodone, methadone, and subutex/suboxone DO show up under their own separate panels. This is not likely to change because it would require a lot more tax payer money in order to use the more expensive multi-panel tests that would actually be able to differentiate.
If it is in your system it like any drug will be detected. If you are using it legitimately it wouldn't matter. Otherwise get off the junk. Mouth swab you need to stop using 3it days before the swab takes place. Also no poppy seeds, cigarettes or coffee the day of the swab as it may register a false positive
Actually the swab test does not go as far back as the urine test. The oral swab test window for opiates is only 1-3 days... usually only 48 hours.

Pain Killers are considered narcotic analgesics and are pretty much all deritives of opium or synthetic opiates. Pain Killers will show up for different lengths of time depending on the type of drug test. Urinalysis tests will show up for up to a couple days. Blood tests will do about the same but a little longer and is more accurate and shows a quantitative value.
