Generally, this is correct. More ideally, place lower 1/3 of screen at eye height. This is actually the center of the most complex information for screen framing (film standard).
Practically speaking, though, this does not work in a group setting (like a meeting room or class room). It would be too low. Average seated eyeheight would be around 48". What works better? Place the horizontal centerline of the screen at about 65" (five foot five inches). Why? Because that's a typical average height of a presenter (in that typical boardroom or classroom), and it will be high enough for everyone in the room (or around the meeting table). As with every "rule", there are exceptions. With K-6, or with a primarily handicapped population, you will want to adjust the centerline of screen to the average seated height of that population, so everyone in the room can see above their associates. But in a Home Theater, where everyone has "front row" access, center of screen (or the lower 1/3 of screen) at the level center of seated eye height.
That depends on the room, your viewing line of sight you're looking to achieve as well as your comfort level for watching TV. Ideally, you should hang your set that you can watch the TV with your head held level, at a comfortable angle, otherwise if it's too high, you'll start to experience neck fatigue.
4ft off the ground to the bottom of tv. I mounted mine 3ft 7 inches and i wish it was just a little higher. too much trouble to remount it. I done remounted it twice. I started out 3 ft 7 inches and went to 3ft, then decided to go back to 3ft 7 inches. If you dont mount it high enough you have to look down too far to see the bottom of the screen. I have 42" tv and I am 24 feet away from it in my bed, and i still have to look down to see the bottom of the screen. I would remount it but my holes are going to get stripped out if i keep messing with it, it already aint getting as tight. So get it right the first time because the more you mount it with the same holes the looser it gets, you have to get bigger screws.

Probably the best method would to be to sit in a chair, sofa, etc. and look straight ahead, or down, slightly. That's the level that's probably the best, just to avoid neck fatigue.
