In cosmetic surgery there is no cutting of skin. A rhinoplasty (or nose job), can be performed nasally, by inserting the tools up your nose. Plastic surgery requires the cutting of the skin to repair or replace what was damaged in an accident or if the child has some kind of deformity when born. It has to do with a total change of the body.
