I was at my Dr. yesterday and was advised that a 5 day pack of Azithromycin stays in your body for 10 days total. So after you take your 5th pill (assuming that you are on the 5 day pack), you have 5 more days of the drug working in your system. She said it has a delayed release effect and that is why it is still effective 5 days after the last dose.
Apparently, Azithromycin accumulates in body tissues more so than other drugs. As a result, it hastens results. Treatment for a common infection that normally would take 7-10 days only takes 3-5 days with this drug.
Reference: "Azithromycin: An antibiotic with unusual properties" in the European Journal of Clinical Microbiology & Infectious Diseases,
The standard answer is that azithromycin is actively fighting infection for 10 days during a course of normal therapy, which includes both the 3 and 5 day dosing of the medication. This answer is based on the drugs long half life, which results from its uptake by phagocytes. (Both 3 and 5 day dosing schedules result in a total of 1.5 grams of azithromycin being delivered to the patient.)
Azithromycin will stay in your system for five half lives. The half life of the azithromycin being one and half days to two days. But if you want to detect the drug, you can detect the same for may be one month.

Yes, there is no problem in taking azithromycin (Zithromax) while on Accutane.
Azithromycin must not be taken by a person who is allergic to erythromycin. The two antibiotics are closely related and work in a similar manner. A qualified health professional will be able to recommend a safe alternative.
