Diazepam can be administered, intravenously, intramuscularly or as a suppository. The onset of action is 1-5 minutes for IV administration and 15-30 minutes for IM administration, although onset can be faster. The duration of diazepam's peak pharmacological effects is 15 minutes to 1 hour for the intravenous or intramuscular route.
Diazepam has a half-life of 20-50 hours, and the main metabolite, nordiazepam, also known as desmethyldiazepam, has a half-life of 30-200 hours. Diazepam is a long acting benzodiazepine. For this reason, it is often substituted for other shorter acting benzodiazepines when dosage reduction or withdrawal is desired. However, with repeated doses, the long half life of the metabolites can accumulate and cause problems with the slower metabolism in the elderly or those with liver disease. It should not be used in patients with severe sleep apnea.
