Only 12 astronauts have landed on the Moon, the first being
Neil Armstrong and Buzz Aldrin (Apollo 11, July 20, 1969).
The others were:
Pete Conrad and Alan Bean (Apollo 12)
Alan Shepard and Edgar Mitchell (Apollo 14)
David Scott and James Irwin (Apollo 15)
John Young and Charles Duke (Apollo 16)
Gene Cernan and Harrison Schmitt (Apollo17, December 1972)
During the NASA Apollo program, 12 astronauts walked on the Moon. There were 2 from each of the 6 missions that successfully landed on the lunar surface.
Another 12 astronauts orbited the Moon but did not land. Three men travelled to the Moon twice during 9 missions. Only one of the three, James Lovell, went to the Moon twice and never walked on it.

In total 14 people have died during space shuttle missions; 7 from the Columbia re-entry desintigration on Feb 1, 2003 and 7 from the Challenger incident where a faulty o-ring caused the shuttle to explode 73 seconds after take-off. NOTE: One of the crew members on Challenger was actually a teacher who one a competition, so technically only 13 astronauts have died in space shuttles.
