Your Veterinarian is the one to discuss this with. It depends on many different things, such as how badly is the heart infected? Are the liver & kidneys affected yet. How is the heart functioning? Is the dog old or young? It is so very important to keep the dog fairly quiet after the medication has been given to kill all the heartworms. If a heavy infection then the dead heartworms can block the heart valves as they come out. The dog need to be quiet for about 4-6 weeks after treatment. Best to talk to the Veterinarian who did the treatment or the one who is going to do the treatment.

If you have ANY dog with heartworm, get them to a vet! Get treatment for them. Heartworms are bad and will cause pain/discomfort/etc for the dog and they will need immediate veterinary care.
