According to the National Association of Jamaican and Supportive Organizations in Kingston, their statistical datas shows that approximately 550 persons are killed in tropical locals by falling coconuts each year. This has caused a new ordinance in most island communities to require coconut tree owners to harvest their coconuts where the trees are over 5 years maturity at least 3 times per year and where the cocunuts are within one month of maturity (where they are more likely to drop from the tree).

The internet if full of reports that falling coconuts kill 150 people worldwide each year, 15 times the number of fatalities attributable to sharks. However, no proof of this statistic can be found even though it is very often quoted by many people.
So sadly, the answer is not really known at this time.
Depends on your proximity to the coconut tree.
it's pretty hard to explain so lets put this way: more people have died from falling coconuts than shark attacks.
Not many, below 1000.
.... That's wrong... first of all 1000 is a LOT! And I have heard it is around 150 per year which is twice the amount of people dying from shark attacks

Aproximately 65 billion people have died since 20 AD
