This depends entirely on the engine in question. Some timing belts/chains have tensioners that automatically set the tension for you. However many more have specific procedures to correctly tension the belt/chain. Do it wrong and it usually ends in tears. If you are changing it yourself you get a workshop manual i.e. a Haynes manual for your car and follow the procedure carefully.
Changing a timing belt/chain isn't usually too difficult, they do tend to be time consuming though as you have to take all the other bits off first to get to the timing gear (hence the high labour charge from garages).
I would always recommend chaning the timing gear (pulleys etc.) rather than just the belt as these all wear together and the cost of something failing is usually expensive
