Through blood they were distant cousins, but through marriage they were first cousins. Wilhelm was the first cousin of Nicholas' wife Alexandra.

More notably, Nicholas's mother's sister was married to Edward VII of Great Britain, who became his uncle. So both Wilhelm II and Nicholas II (and Nicholas's wife) were first cousins of king George V of the United Kingdom.
