Slowly and painfully. The bones in the legs are cut apart, then a metal frame is assembled around the leg, with pins going through the skin and into the bones on both sides of the break. This metal frame is then extended a little each day, pulling the parts of the bone further and further apart from each other. As the body is trying to close the gap the bone will lengthen.
