It is possible for a child to get a fever after being on an antibiotic for three days. There are several reasons. A few of those reasons are that the child could be sick with a virus and not a bacteria, in which case the antibiotic would be useless. Another scenario is that the bacteria is resistant to the particular antibiotic and that a different one is needed. In either case, you should follow up with the child's physician.

If your child has been prescribed an antibiotic and continues to run a high fever (> 103 F), one of two things are going on: 1. either the bacterial infection for which the antibiotic was prescribed is not taking effect or 2. the child has a concomitant viral infection, in which only symptomatic care would suffice. Remember, antibiotics do not cure viral infections.

I'm not a doctor, but I'm a mom. If the child's on antibiotics then he/she is being treated for a bacterial infection. If the child develops a viral infection, that might cause a fever. Viruses are not responsive to antibiotics. (Antibiotics are for bacteria, viruses are from a "bug" -- NOT bacteria). So yes, a virus would cause a fever even if she/he is on antibiotics for a bacterial infection.

If there is no improvement in two to three days after starting an antibiotic the child should be reviewed by a doctor.
