You create one copy of the document and leave a space at the bottom for your parents' signatures and for a notary statement/signature/stamp. Fill it out and have your parents get it properly notarized. Your Aunt does not need to sign it. Give the document to your Aunt. She will have to carry it with her and it will serve as her parental identification whenever requested.
