It is mostly a random (and relatively common) occurrence, unless it is of the translocation type which is rare and genetic. The exact causes of Downs Syndrome are not known clearly but Downs tends to occur with older (36+) mothers. It does not seem to have any direct association with drugs but drugs can certainly do many types of harm including reproductive damage.
